Responsibilities
Ensures health and safety of the individual, including personal care.
Provide education, training, counseling, and physical assistance when necessary to achieve ISP goals in functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self‑esteem, and money management.
Communicate with family, friends, guardians, advocates, volunteers, employers to facilitate consistency of programming and support.
Develop, write, and present annual ISPs and semi‑annual agreements in consultation with other residential staff, Program Manager, and Director.
Perform case management duties.
Provide transportation to individuals as necessary.
Schedule and supervise staff.

Job Requirements
BA/BS in Human Services or related field or substitute 10 years of related experience with persons with ID/DD or human services.
Effective oral and written communication skills. All candidates will complete a 1‑page writing sample during the interview process.
Personal vehicle.
Valid driver’s license in state of residency.
Certifications & Training
CPR & First Aid
Medication Administration Certification
All candidates must pass a CORI, reference, multi‑state background, and driving record check.
