This position is part of the ambulatory care pharmacy team and focuses on cardiac disease state management within a hospital‑based clinic. The specialist collaborates with cardiology providers to optimize outpatient pharmacotherapy for conditions such as heart failure, arrhythmias, ischemic heart disease, with a focus on cardiomyopathies. This role includes leading clinic initiatives, supporting transitions of care, and educating patients, providers, and pharmacy trainees. Daily activities include direct patient care, medication management, and precepting pharmacy residents and/or students. Non‑direct patient care activities may include cost savings initiatives, committee involvement, and participation in departmental projects and practice advancement efforts. Benefits

Education: PGY1 and PGY2 from an ASHP‑accredited residency program; PGY1 with 5 years of progressive clinical practice experience; Bachelor’s Degree of Science or PharmD from an ACPE‑accredited school of pharmacy. Experience: 5 years progressive clinical practice experience within specialty with PGY1 from an ASHP‑accredited residency program or 8 years progressive clinical practice experience within specialty. Computer Skills and Data Entry Experience

BASIC LIFE SUPPORT certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Requirement for REG PHARMACY TECNICIAN certification within 30 days of hire; TSBP license acceptable; ACLS certification required for inpatient positions within 6 months of employment. English (speaking, reading, writing). Good general health and stress coping ability. Moderate physical effort: prolonged standing/walking, occasional lifting/carrying (up to 25 lbs.), reaching, bending, crouching, etc. Job Duties

Performs job responsibilities in accordance with organizational and departmental policies and procedures, performance improvement objectives and environmental/infection control standards. Develops/implements process to meet/comply with JCAHO and regulatory requirements. Coordinates staff training, competencies, certification and continued education specific to specialty. Assists clinical staff pharmacists with therapeutic monitoring, patient assessments, interpretation of lab values, TPN assessment and pharmacokinetic calculations. Identifies opportunities for process improvements, develops and implements policy and procedures for prompt, safe and efficient patient care. Coordinates multidisciplinary in‑services/meetings, publishes articles, provides individualized drug information, rounds, consults, and maintains high visibility/availability through participation in in‑services, meetings and daily activities. Participates in the education/experiential training of pharmacy students and pharmacy residents. Manages P&T initiatives through monitoring of documented interventions, verifies data, develops measures and reports outcomes (Performance Improvement). Supports cost savings initiatives and has active role in assessment and selection of cost‑effective pharmaceuticals. Reviews formulary addition request and develops monographs for presentation; manages the formulary system and procedures for acquisition of non‑formulary product. Mentors clinical staff pharmacists and supportive personnel. Delegates appropriate/reasonable tasks and evaluates performance. Represents Pharmacy Department at P&T Committee and Service Line Meetings. Performs computer order entry accurately, thoroughly and timely. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Actively enhances personal growth and development by participating in educational programs, literature searches, in‑services and meetings. Actively participates in new employee orientation and training and evaluations. Handles responsibilities that include directly interacting with or caring for patients, maintaining or modifying patient records, and maintaining confidentiality in accordance with policy. Takes ownership in solving problems effectively, efficiently and to the satisfaction of the patient. Presents constructive feedback regarding system or process failures and participates in resolution in a proactive and positive manner. Maintains patient confidentiality, adheres to all policies and procedures related to safety, infection control, and corporate compliance. Responds to all reasonable requests in an appropriate and timely manner and communicates in a concise and positive manner.
