Overview
RN Clinical - 4 S ICU - Full Time 12 Hour Days (Non-Exempt) (Union) Responsibilities
Follows policies and procedures; demonstrates ability to access online manuals. Transfers learned scientific knowledge into practical application in applying the Nursing Process. Assesses patient status upon admission, at each shift, and with change of condition. Completes patient acuity by 0200/1400 daily. Diagnoses by interpreting assessment information and formulating nursing diagnoses. Plans of care with individualized interventions. Documents expected outcomes. Reviews/updates plans of care daily based on evaluation of patient outcomes. Administers medications and IV therapy appropriately at prescribed intervals. Delegates tasks based on scope of practice and competency. Acts as patient advocate. Manages restraints per policy and uses non-restraint measures first. Ensures MD orders and assessments per policy. Ensures patient monitoring and completes Restraint Audit Form each shift. Documents on all flow sheets and chart forms: Admission Assessment, Pain scale assessments, Daily flow sheet, Restraint flow sheet, MAR, Care Plan, risk assessments, and other required records. Ensures orders are transcribed and signed off prior to end of shift. Completes 12h/24h chart checks. Provides patient/family education as needed. Includes room/unit orientation and Plan of Care. Identifies knowledge deficits related to diagnoses and care. Uses MICROMEDEX information when appropriate. Transfers and discharges: collaborates with Case Manager/physician regarding discharge planning; ensures transfer paperwork and belongings are completed. Utilizes patient care equipment safely and according to manufacturer guidelines (IV pump, feeding pump, PCA pump, Epidural pump, SCDs, Wound VAC, heating devices). Communicates effectively with the healthcare team and patient/family: shift reports, rounds, hand-offs, and related communications. Manages time effectively and prioritizes responsibilities; adheres to schedule and unit expectations. Observes regulatory agency requirements and patient safety goals, including patient identifiers, universal protocol, medication reconciliation, clinical alarms, HIPAA/OSHA requirements, and survey readiness. Maintains safe practice in the work environment, including body mechanics, electrical safety, and infection control procedures. Demonstrates ability to manage care for specified medical-surgical, ICU, tele-metry, dialysis, psychiatric, and transplant patient scenarios as appropriate to unit assignment. Engages in quality measures and professional development activities; participates in data collection and process improvements; may serve on committees and unit projects. Performs other related duties as assigned. Qualifications
Req Bachelor’s Degree in Nursing (BSN) — grandfathered from BSN if hired prior to 12/2019 Req Graduated from an accredited school of nursing Req 1 year acute inpatient hospital experience as RN Req Excellent communication skills in English (speaking, reading, writing) Req Knowledge of medical procedures and conditions; ability to perform duties per policies and procedures Req Commitment to excellence in patient care and customer service Req Ability to work independently with minimal direction Licenses/Certifications
Req Registered Nurse - RN (CA Board of Registered Nursing) Req Basic Life Support (BLS) Healthcare Provider (American Heart Association) Req Fire Life Safety Training (LA City) — if no card, obtain within 30 days of hire (LA City only) Req Advanced Cardiovascular Life Support (ACLS) (American Heart Association) Pref Specialty Certification The hourly rate range for this position is $44.00 – $95.00.
