Job Purpose The Dementia and Frailty Lead Nurse will provide clinical leadership for our Dementia and Frailty Service, supporting the Nurse Director in delivering strategic direction, driving service improvements and acting as an expert clinician to aid decision‑making across a range of health and care professions.

Key Responsibilities

Provide strategic leadership and support to the Dementia and Frailty Service.

Drive service improvements and best practice in dementia and frailty care.

Act as an expert clinician for decision‑support across professions.

Collaborate with professional and organisational partners, engaging a wide range of stakeholders.

Represent the service in clinical governance and quality improvement initiatives.

Qualifications & Experience

Registered Nurse with relevant experience in dementia and frailty care.

Experience working in remote and rural settings is desirable.

Must be registered with the PVG Scheme (Protecting Vulnerable Groups).

Strong communication, leadership and organisational skills.
