Job Definition The Certified Peer Specialist functions as a member of the Intensive Crisis Stabilization Center to assist with the outreach, support and coordination of care for eligible Nassau County residents. The Peer Specialist will serve as a role model and/or health coach for clients.

Educational Requirements

High School Diploma or equivalent.

Peer certification (NYCPS, CRPA, or FPA).

Experience Required Relevant life experience is required: personal experience living with a disability and/or social/emotional challenge, and willingness to draw upon this experience to serve as a role model and support the recovery model within the Health Home model.

Duties and Responsibilities

Provide health coaching and recovery support, especially on issues of self-advocacy, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.

Maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.

Coordinate care planning with other providers of services/resources to ensure goal‑directed, collaborative care, including care transitions.

Provide crisis intervention.

Facilitate sharing of data with individuals to support partnered decision‑making and keep clients informed of progress.

Complete QI training during onboarding and participate in QI refresher training as needed.

Cultivate a culture of inclusion for all employees that respects individual strengths, views and experiences, driving better decisions, innovation, and business results.

Adhere to all safety protocols and procedures to maintain a safe work environment for all employees.

Demonstrate commitment to maintaining a safe work environment by following established safety guidelines.

Show knowledge or experience regarding health and safety protocols to ensure safety of all employees.

Perform other duties as assigned.
