Location and Schedule Health Center Address: 400 W New Rd, Greenfield, IN 46140

Schedule: Monday, Tuesday & Friday 7 a.m. – 4 p.m.

Day in the Life As a provider you practice relationship‑based medicine, offering holistic care—prevention, chronic disease management, and health education. You have access to resources like UpToDate and RubiconMD. You work autonomously in a patient‑first environment.

Minimum Job Requirements – Nurse Practitioner

Active license & current ANCC or AANP board certification

Independent practice provider preferred (per state regulation)

Federal DEA number preferred (may be required for full prescribing)

CPR/BLS certification required at start date

Independent family practice experience (routine wellness, chronic care, urgent visits) preferred

Minimum Job Requirements – Physician Assistant

Active license & current NCCPA board certification

Pay & Compensation Salary range: $82,000 – $112,000 per year for a 30‑hour weekly schedule. Actual offer may vary based on geographic location and experience.
