This is a professional position responsible for providing all access/intake services within the Developmental Disabilities division.

Organization:

The Outreach & Intake Specialist reports to the ID/DD Supports Coordination Supervisor and supervises no staff.

Essential Functions:

Conducts screenings for services, assessments and initial referrals to community-based service and support needs (various medical evaluations, etc.) and coordinates/follows-up on referral process. Gathers and analyzes intake evaluations to determine eligibility. Provides initial supportive counseling/consultation to clients and families Completes initial screenings for the DD Waiver Wait List and updates information for individuals on the Waiver Wait List on an annual basis and completes assessments and reports for the Waiver Slot Allocation Committee. Monitors individuals assigned to the DD waiver waitlist and provides periodic case management support as needed. Provides Lead Agency Case Management services to individuals diagnosed with a Developmental Disability who are receiving CSA/FAPT funding. Coordinates special outreach projects/ community presentations and other assignments as needed. Provides in-service training, community trainings on Developmental Disability Services. Partner with community partners such as the local school system and other local organizations to provide community presentations. Supports activities of Hanover Community Support Services, as needed. Provides backup coverage to support coordination staff as needed. Performs related work as assigned. Working Conditions: A. Knowledge, Skills and Abilities : Requires working knowledge of marketing/public relations principles and skills in networking; extensive knowledge of Medicaid, DD waiver, and Dept. of Behavioral Health & Developmental Disabilities (DBHDS) licensure and human rights regulations/standards and service delivery systems for persons with mental illness, intellectual or developmental disabilities, and substance abuse disabilities - including community based services and community resources, implementation of service plans, and prevention/early intervention theories and concepts. Must be conversant in Person-Centered Planning practices. Ability to communicate effectively both orally and in writing, required. Must be able to interpret/apply regulatory and funding agency guidelines/objectives. Must be able to develop and maintain effective work relationships with staff, businesses, churches, government and community agencies. Ability to maintain a high degree of confidentiality, exercise initiative, sound judgement, while working independently. Must have ability to conduct training and to provide community education.

Education, Experience and Training : Bachelor's degree required preferably in related field, with at least five (5) years' related experience - OR - Any equivalent combination of education, experience and/or training sufficient to demonstrate the knowledge, skills and abilities is acceptable. Master's degree preferred.

Special Conditions: • Criminal Records Check, including fingerprinting • Must obtain valid Commonwealth of Virginia Driver's License within 30 days of employment to be eligible to operate a county vehicle • CPS (Child Protective Services) Check • Twelve-month probationary period • Work beyond normal work schedule • Must be able to successfully pass CPR/First Aide training as well as behavior management as required by regulation, including but not limited to Title 12 of the Virginia Administrative Code. This training process includes a hands-on, in-person demonstration of first aid and CPR competency for successful completion. This demonstration requires bending, lifting, and completing activities on the floor.
