Base pay and location Base pay range : $24.04/hr - $28.85/hr
Salary Range: $24.04 to $28.85 per hour, based on relevant years of experience.
Location: Larimer County, Colorado
Status: Bristlecone onsite 3 days a week and 2 days remote
Role overview Responsible for coordinating integrated (i.e. substance abuse and mental health) case management services for our adult or children\'s populations. Case managers engage, assess, plan, advocate, coordinate and link clients to formal/informal activities, services, and/or support within the community that promote wellness and safety. Case managers provide individual, group, family, and crisis therapeutic interventions. Case Managers develop and monitor service plans, document interactions, maintain clinical records, and other necessary data reporting for adult and child consumers/clients with co-occurring disorders and/or disabilities in Larimer County. Case managers coordinate not only with external collaterals in the community, but internal treatment and medical teams as well. Position will include individual orientations and coordination of our SUD IOP program.
Essential duties
Coordinates a wide variety of services for clients to receive benefits from internal and external service providers regarding housing, social security, etc.
Provides assistance and coaching to client/consumers with daily life activities such as bill paying, managing finances, expense tracking, completing applications for services, and other needs which may include acting as client\'s Social Security payee.
Educates consumer/client on behaviors to maintain daily life activities.
Interacts regularly with federal, state and local entities to create opportunities to deliver services to client/consumer. Represents clients/consumers with these entities to get services, amend services, or re-instate services.
Performs case management services as needed to help clients meet goals stated in their service/treatment plans. Refers clients/consumers to supportive services such as vocational, medical, social security, housing authority, etc.
Assists consumers in improving their well-being and quality of life by coaching in behavioral skills; educates consumers to make informed choices; develops clients\' coping skills; helps consumers to problem solve and negotiate tasks of daily living.
Facilitates groups.
Documents progress notes, case management notes, letters, faxes and phone calls. Maintains all required paperwork for payee ships.
Maintains close and cooperative relationships with community agencies and other public/private entities (landlords, banks, etc.) to carry out supportive services.
Advocates for consumers in various forums including federal, state and local systems, with family and school systems.
Performs other duties as assigned.
Required qualifications
Relevant bachelor’s degree (e.g., in social work, human services, or related field)
Being on the CAS track will be required
1–3 years minimum of directly relevant experience in the Mental Health or Substance Use Disorder field
Experience with mild to severe mental illness and substance use disorders
Experience in coordinating services with other agencies
Preferred qualifications
Master’s degree in social work or other human services/health-care related field
Bi-lingual/bi-cultural preferred
Some positions include driving a company vehicle and require a valid Colorado driver’s license.
