Nurse Practitioner - NP/PA Primary Care

Join us at our Baytown clinic for a full‑time (40 hours/week) position.
Salary Range:
$109K-$154K based on experience plus quarterly incentive plan.
Responsibilities
Provide primary and family evidence‑based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients.
Provide administrative duties including opening/closing clinic, inventory, patient follow up, verifying insurance, collecting payment, and maintaining a clean environment.
Handle patient appointments, perform routine examinations, ensure positive health outcomes, and coordinate care with other healthcare professionals.
Encourage patients to become active participants in their healthcare through screenings, behavioral health, wellness, and chronic condition management.
Required Qualifications
Minimum one year experience as a Nurse Practitioner or Physician Assistant; experience in Family medicine or primary care preferred.
Active Family Nurse Practitioner or Physician Assistant license in good standing.
Ability to provide evidence‑based patient care including pregnancy prevention, STI prevention, and vaccine counseling.
Order and administer vaccinations and injections (COVID‑19, influenza, TB, Depot).
Minimum two years medically relevant experience or equivalent.
Provide DOT certification within 30 days of employment.
Proficiency in at least one additional language (e.g., Spanish) preferred.
Basic Life Support (BLS) certification; ability to perform CPR and use AED.
Pass a FIT test and wear respirator mask when required by protocol.
Education
Master’s Degree in Family Nurse Practitioner with current National Board Certification and state license for Advanced Practice Nurse role.
Or Master of Science in Physician Assistant Studies (or equivalent) with current National Board Certification and state license.
Preferred Qualifications
EPIC experience.
Pay Range Typical range: $95,738 – $206,206. Eligible for CVS Health bonus, commission, or short‑term incentive programs.
