RN Case Manager - Barnes-Jewish Hospital
Pay Range: $32.80/hr – $50.39/hr; Sign‑on bonus: $1,250 (external candidates). Benefits eligible. Part‑time (24 hrs per week). Onsite. Schedule: Monday–Friday, 3 shifts per week, day shift 8:00 am–4:30 pm, 1 weekend day per month. Holiday rotation.

The Case Management Service reports through the division of Patient Care Services and focuses on ensuring all patients have access to safe discharge planning and continuity of care.
Role Purpose Coordinates, negotiates, procures, and manages patient care to achieve quality, cost‑effective outcomes. Works collaboratively with physicians and interdisciplinary staff to identify and resolve issues and provides consultation regarding Utilization Management and Performance Improvement programs.
Responsibilities
Assesses, coordinates, and evaluates each patient's plan of care.
Establishes, continuously evaluates, and documents an individualized discharge plan.
Ensures clinical documentation and cost‑effective use of hospital resources.
