Care Manager (CNA)
Full-time, 2:30 pm–10:30 pm or 10:30 pm–6:30 am (32 hour positions in Assisted Living and/or Memory Care).
Led by the community's Director of Health & Wellness, we are looking for compassionate and dedicated caregivers (Care Managers) to assist residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.
Responsibilities and Duties
Provide resident care as outlined in their care plan, including bathing, dressing, toileting, and feeding.
Maintain the safety, comfort, dignity, and confidentiality of all residents.
Escort residents to meals and activities.
Ensure all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts.
Document all services provided during your shift.
Report any change of condition in a resident immediately to your supervisor.
Qualifications
Certified Nursing Assistant (CNA).
Knowledge and experience working with the elderly population, preferred dementia experience.
Physical Abilities
Supervise duties may require standing, walking, sitting, using fine and gross motor skills, reaching, balancing, stooping, kneeling, crouching, talking, hearing, and smelling.
Must be able to lift or carry weights up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. Proper body mechanics must be used.
