Psychosocial Assessment – performs assessments considering family, social, and medical history; maintains knowledge of diagnoses; reports suspicions of abuse and neglect; completes accurate documentation. Resource Support – collaborates with the health‑care team to develop treatment plans and identify appropriate resources; provides real‑time accurate information to patients/families; evaluates effectiveness of interventions and makes adjustments as needed. Psychosocial Interventions – provides supportive counseling to improve emotional coping and treatment adherence; educates patients/families and team on treatment options; offers self‑management support for high‑risk/complex patients; conducts crisis assessment and interventions; facilitates family‑centered care team meetings. Interdisciplinary Collaboration – advocates for patient/family needs within the health‑care system; acts as liaison among medical team, patients, families, and outside agencies; empowers patients/families to problem‑solve care options; coordinates communication to minimize fragmentation of services. Collaboration – partners with patients, families, and the health‑care team during safe transitions to the next appropriate level of care; educates health‑care professionals on psychosocial aspects and the role of social work; promotes a positive work environment; participates in professional development opportunities. Job Qualifications
Master’s degree in Clinical Counseling or Social Work. 2+ years of related work experience. LPCC or LISW license in the state of Ohio. May require clinical license, certification or training. Location
