Responsibilities
Provide service coordination (case management) for assigned consumers in the community and offer on-call crisis coverage.
Conduct comprehensive assessments and oversee ongoing monitoring of mental illness symptoms.
Act as liaison and consult with community agencies and families to maintain coordination throughout the treatment process.
Coordinate with psychiatric inpatient services and detoxification providers to ensure appropriate length of stay, and facilitate linkage to outpatient self‑help programs and residential facilities.
Provide individual and group substance‑use counseling.
Offer ongoing assessment, problem solving, skill training, supervision, and environmental adaptations to assist clients with activities of daily living (ADLs).

Qualifications
High school diploma or equivalent; bachelor’s degree in social work, psychology, sociology or related field preferred.
Minimum 1 year of experience in drug and alcohol counseling.
BCLS certification upon completion of hospital orientation.
Bilingual Spanish speaking (preferred) and ability to work with culturally diverse populations.
Hourly range:
$17.26–$22.44
