Speech Language Pathologist – Nuvance Health

This position assesses and evaluates speech, language and/or hearing disorders, determines and makes recommendations to physicians for corrective treatment, and collaborates in interdisciplinary care planning. The role supports patient and caregiver communication, monitors progress, and contributes to quality improvement initiatives within a home‑health setting.

Responsibilities

Assesses client needs using diagnostic speech and language evaluations and recommends appropriate corrective treatment to the physician.

Participates in the development of the individualized care plan and case management, in consultation with the physician.

Maintains accurate records of visits, diagnostics, plans of care, client responses to therapy, and discharge summaries.

Attends case‑management conferences with nurses and other team members to coordinate the speech‑therapy program with the overall care plan.

Reports to the physician the client’s response to treatment, changes in condition, progress toward goals, and reassesses at least every 60 days.

Instructs families and team members in techniques that assist clients in improving speech and swallowing functions.

Provides written instructions and demonstrations to home‑health teams, ensuring documentation is available in the client’s record and on site.

Participates in the agency’s quality‑assurance and performance‑improvement plan.

Supervises care provided by home‑health aides as appropriate.

Complies with all regulatory responsibilities, including attending required in‑service training.

Performs additional duties as assigned.

Education & Qualifications

Master’s level education in Speech‑Language Pathology (DGRE).

Graduated from an ASHA‑approved curriculum and successfully completed the National Registration Examination.

For international graduates, successful completion of the registration examination as prescribed by ASHA. A supervisor must be a licensed Speech‑Pathologist with at least three years of clinical experience.

Evidence of one year of full‑time, paid speech‑therapy experience in a hospital, clinic, or agency under close supervision.

Minimum experience: one year.

Desired: Functional Independent Measurement (FIM) certification, CPR certification, and experience with FEES or MBS.

Working Conditions

Manual: Some manual skills/motor coordination and finger dexterity required.

Occupational risk: Some occupational risk present.

Physical effort: Medium to heavy effort; may exert up to 35 lbs. force.
