Admissions Liaison : Acute Rehab Nursing

Responsibilities

Proactively identify clients who meet program criteria and manage the assessment admission process into the Fudge Family Acute Rehabilitation Center.

Provide education on acute rehabilitation services and resources to patients, case managers, physicians, and outside agencies, including participation expectations and hours of therapy per day.

Assist patients with the admission process to the rehabilitation hospital.

Determine eligibility by verifying coverage information, ensuring IRF criteria met and securing physician acceptance/approval via pre‑admission screen documentation.

Facilitate benefit eligibility and pre‑certification, including plan of care and external payer negotiations.

Serve as liaison case manager with internal and external case managers.

Maintain optimal unit census through coordination of admissions and discharges while compliant with the 60/40 CMS rule.

Develop strategies to grow market share.

Provide data management and outcome analysis.

Perform other duties as assigned.

Qualifications

Bachelor’s Degree required.

Previous experience with multiple payers and insurance required.

Two years of clinical experience with preference for acute rehabilitation.

Experience with Medicare IRF admission criteria preferred.

Bilingual or multilingual preferred.

License Required

Current California licensure: RN, PT, OT, SLP, or LCSW.

Valid California driver’s license.

Certifications Required

BLS

Certifications Preferred

Certification in Rehabilitation Nursing
