Job Description Provide high quality, patient centric, cost effective in‑patient level of care in the home setting. This is a virtual position based out of our Command Center in Charlestown, MA.
Clinical outcomes are equal to or better than inpatient hospitalization.
Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center.
Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post‑acute care, and readmissions.
Improved patient experience.
State‑of‑the‑art equipment at Command Center and patients’ home.
Patients have home infusion, meals/nutrient consults, pharmacy/medical mgmt., mobile imaging, laboratory and DME/oxygen.
Integrated care with primary care providers, specialists & ancillary departments.
Minimum Qualifications
BC/BE Internal Medicine/Family Medicine/Geriatrics licensure, certification and registration.
Skills, Knowledge & Abilities
Experience as a hospitalist or SNF a plus.
Committed to providing high quality, evidence‑based medicine to a diverse patient population.
Excellent clinical and communication skills.
Demonstrated ability to work in a team environment.

Pay Range $240,000.00 USD - $316,000.00 USD
The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation may be determined based on several factors, that may include education, training, relevant experience, specialty, geography of work location, productivity, job responsibilities, or other applicable factors permissible by law. In addition to base compensation, this position may be eligible for performance‑based incentive compensation, which may include productivity and quality bonuses.
