Job Description Provide high quality, patient‑centric, cost‑effective in‑patient level of care in the home setting.
Clinical outcomes equal to or better than inpatient hospitalization.
Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center.
Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post‑acute care and readmissions.
Improved patient experience.
State‑of‑the‑art equipment at Command Center and patients’ home.
Patients have home infusion, meals/nutrient consults, pharmacy/medical management, mobile imaging, laboratory and DME/oxygen.
Integrated care with primary care providers, specialists & ancillary departments.
Minimum Qualifications
BC/BE Internal Medicine / Family Medicine / Geriatrics.
Skills, Knowledge & Abilities
Experience as a hospitalist or SNF a plus.
Committed to providing high quality, evidence‑based medicine to a diverse patient population.
Excellent clinical and communication skills.
Demonstrated ability to work in a team environment.
Pay Range $120,000.00 USD – $158,000.00 USD
