Clinical Pharmacy Specialist

Implements, coordinates, and evaluates all clinical Pharmacy services. Assists and directs Medication Usage Evaluations, collaborates with the medical staff, and establishes criteria for the safe, effective, and appropriate use of medication. Job Responsibilities Develops and monitors hospital policies and procedures regarding medication utilization in collaboration with the medical staff, Pharmacy, and Therapeutics Committee; ensures effective medication utilization and intervenes when necessary for compliance. Monitors pharmacists' activities related to utilization surveillance, provides instruction and education as needed, maintains records on drug therapy effectiveness, and supplies information as required. Ensures safe and effective medication use; guides staff in providing clinical services. Provides information to management for staff performance appraisals; coordinates and implements educational training and competency programs for staff and pharmacy students. Provides educational programs on medication use for healthcare staff; measures success by staff's ability to meet goals and improve patient care; incorporates professional concerns into the programs. Coordinates activities with student preceptors, monitors student performance, and initiates programs. Participates in developing and reviewing the hospital's Formulary and related activities. Initiates and coordinates Medication Use Evaluations (MUEs) for P&T presentations and discusses findings. Performs all duties of a licensed registered Pharmacist as required, maintaining compliance with procedures, policies, and laws. Performs related duties as required, considering all essential functions under the Americans with Disabilities Act. Job Qualifications Bachelor of Pharmacy with 1-year internship or equivalent education and experience.
