Job Title: Physical Therapist (PT) or Occupational Therapist (OT) - Lymphedema Care
Location: Region J - Titusville, Cocoa, FL
Type: Full-Time (Day Shifts)
About the Role: We are seeking a licensed and motivated Physical Therapist (PT) or Occupational Therapist (OT) to join our in-home chronic disease management program, specializing in lymphedema care. In this role, you will deliver individualized, evidence-based therapy to help patients with lymphatic disorders regain mobility, function, and independence.
Key Responsibilities:
Conduct comprehensive assessments for patients with lymphedema or lymphatic dysfunction. Develop and implement personalized treatment plans, including manual lymphatic drainage, compression therapy, exercise, and functional interventions. Provide direct, hands-on care and supervise assistants (PTAs or COTAs) as applicable. Collaborate with interdisciplinary team members to ensure optimal patient outcomes. Maintain accurate, compliant, and timely documentation. Adhere to infection control and safety protocols. Qualifications:
Graduate of an accredited Physical Therapy or Occupational Therapy program. Current Florida licensure as a PT or OT. Minimum 1 year of home health experience (required). Lymphedema Certification (CLT) required. Current CPR certification. Valid driver's license and reliable transportation. Strong documentation and computer skills. Bilingual abilities are a plus. Monday to Friday, daytime hours. No on-call or night shifts. Occasional weekend visits may be required based on patient needs. Caseload:
24-30 visits per week (approximately 5 patients per day). Work Setting:
In-home chronic lymphedema program (non-traditional home health). Territory Coverage: Clinicians are typically assigned to a primary zone within their county of residence, plus a nearby secondary zone. Travel to nearby counties or cities may occasionally be required to maintain a full caseload.
