The Community Outreach Coordinator will spearhead outreach and retention initiatives for the WIC program, increasing community engagement through culturally relevant information and events. This professional will collaborate with BMS WIC staff, other sponsoring agencies, and community partners to create materials that overcome enrollment barriers and encourage participation. The coordinator will deliver Public Health Detailing to enhance provider relationships, facilitate warm hand-off services, and cultivate a robust referral network with healthcare providers and community-based organizations.
Additionally, the coordinator will support healthy lifestyle initiatives, including Farmer's Market Nutrition Program activities. Alongside outreach responsibilities, the coordinator will serve as a Nutrition Assistant and Program Support staff as needed to ensure seamless operations. Key responsibilities include maintaining outreach logs, documenting activities in the management information system (NYWIC), assisting with quality performance reports, and tracking demographic data.
The coordinator will participate in community events, conduct BMS quarterly site visits, and advocate for the program and health center by backing outreach initiatives and lobbying for their interests. Daily tasks include maintaining outreach activity and referral logs and documenting outreach and retention efforts in NYWIC for state reviews.
Essential Functions and Duties:
Manage all outreach and retention efforts in coordination with the WIC Director.
Assess community needs to identify unmet areas and underserved populations.
Monitor outreach and retention activities logged in the NYWIC database.
Evaluate the effectiveness of outreach strategies and collaborate with the WIC Director to modify future plans as necessary.
Support the Nutrition Coordinator & Health & Wellness Educator in planning health promotion activities for the WIC Program and health center.
Make appropriate referrals and document follow-ups with other health and social services for participants.
Develop and implement outreach and retention plans while ensuring timely report submissions and data collection.
Create a comprehensive directory of health care and human service providers to facilitate referrals.
Conduct public health detailing to ensure providers understand the WIC program and encourage referrals.
Lead outreach and nutrition workshops within communities to raise awareness of healthy lifestyles, including promoting breastfeeding.
Organize outreach activities for the WIC Program, health center, and the wider community, providing valuable eligibility information on WIC benefits.
Deliver presentations, attend community meetings and health fairs, and act as a linkage/referral liaison for the WIC Program.
Perform the duties of Nutrition Assistant & Program Support staff as required, ensuring adequate support for clinic and office operations.
Conduct both remote and in-person services as necessary.
Required Skills/Abilities:
Outstanding communication and writing skills, with experience working alongside Community-Based Organizations or health centers. Must work effectively independently.
Education and Experience:
Advanced degree with at least one year of experience in the community.
Bachelor's Degree is preferred.
Physical Requirements:
Must be able to lift up to 25 lbs.
