Oncology Clinical Pharmacist Position Details: Department: PCH-MAIN | Pharmacy Location: Phoenix Shift: Mon-Fri, Sat, Sun, (Days Vary), Shifts Vary, 6:30am-12:30pm Category: Pharmacy Employee Type: Full-Time Position Summary Join our dedicated team as an Inpatient Clinical Pharmacist specializing in pediatric oncology at our Thomas Campus. This vital role requires extensive experience in pediatric pharmacy and a thorough understanding of oncology protocols to manage complex medication regimens for children undergoing cancer treatment. You will have a direct impact on our patients' care in an inpatient setting. Position Highlights: Schedule: 8-hour shifts with opportunities to work decentralized in the oncology unit, allowing for direct contribution to patient care. Requirements: Pediatric pharmacy experience is necessary; pediatric and oncology experience is preferred. Completion of an ASHP-accredited PGY1 pharmacy residency is desirable. Position Duties Review and manage medication orders for therapeutic appropriateness, taking into account age, weight, height, laboratory values, diagnosis, and other pertinent clinical information. Effectively screen for and address clinical alerts/conditions, such as allergies, interactions, therapeutic duplications, and adverse drug reactions (ADRs). Proactively resolve medication order issues and consult with prescribers to recommend appropriate alternative therapies as necessary. Adhere to the directives of the Pharmacy and Therapeutics Committee regarding Formulary Management. Document clinical interventions to meet established cost containment and evidence-based medication therapy goals for the department and organization. Prepare and dispense sterile and non-sterile medications accurately and promptly, in compliance with departmental policies, annual department goals, and all applicable laws and quality standards.
