Introduction THIS RECRUITMENT IS LIMITED TO CURRENT EMPLOYEES OF THE MDH WORCESTER COUNTY HEALTH DEPARTMENT ONLY.

Location of Position MDH Worcester County Health Department 6040 Public Landing Road Snow Hill, MD 21863

Main Purpose The main purpose of this full‑time position will be to provide community based case management.

Responsibilities

Identify the intervention needs of children/adolescents and adults on the caseload.

Perform assessments.

Provide recommendations and connect individuals with appropriate resources to obtain a more satisfactory social, economic, emotional and/or physical adjustment.

Operate vehicles of various sizes to transport clients and make home visits.

Some weekend and/or evening hours may be required. The selected applicant must be willing and able to travel to all Worcester County Health Department sites.

Minimum Qualifications Education : Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience : One year of administrative staff or professional work in a health care setting.

Notes

Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year‑for‑year basis for the required experience and education.

The evaluation may be a rating of your application based on your education, training and experience as they relate to the requirements of the position.
