JOB SUMMARY The CCHIP Case Management (CM), Bilingual will provide client-centered, confidential services that link clients with health care, clinical psychosocial, and supportive services for clients living with HIV/AIDS who face challenges with accessing and maintaining adherence to health care services. Works closely with the CORE Center medical team to stabilize clients’ medical status. Facilitates linkage to and maintenance of clients to their primary medical services. Provides treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments. Ensures clients connect to other core services (dental, Mental Health, and Substance Abuse treatment). Provides benefits counseling to clients and assists in enrollment, verification, and utilization of those benefits. Provides case management to special populations.

This is a grant-funded position. Grant - CCH Ryan White pt B Reentry Region 8 will expire on June 30, 2026, with the potential to be renewed.

TYPICAL DUTIES

Completes initial intake and assessment of needs on new clients and clients returning to care.

If opened for CM services, complete AIDS Foundation of Chicago (AFC) intake documentation.

Make all initial referrals to Behavioral Health, Health education, dental, and benefits.

Assess benefits and complete necessary applications and referrals.

Maintain a caseload based on funder’s requirements.

Develops a comprehensive, individualized service plan.

Reviews with client; client will agree to the goals and objectives for the service plan with specific outcomes and timelines.

Conducts periodic re-evaluations at least every 6 months to review progress, continuation, or modification of goals.

Collaborates with medical provider and other clinic staff to assure compliance with the service plan.

Maintains contact with client to assure medical and medication adherence.

Contacts client prior to scheduled medical appointments to remind them of the appointment.

Performs face-to-face contact with client at least once every three months and maintains monthly phone contact in between.

Conducts outreach if non-compliant (phone outreach, letter, and, if needed, home visit) to encourage adherence.

Advocates for client education on clinic appointments, medication adherence, and referrals to Substance Abuse and Mental Health Services.

Identifies barriers to appointments and provides support to encourage adherence (e.g., transportation).

Makes referrals and documents in electronic databases; ensures timely follow-up.

Meets with medical providers regularly to support client’s appointment compliance.

Presents during scheduled medical appointments to advocate for identified services.

Attends pre-clinics to update on client’s social service needs and collaborate with providers.

Completes a CM care conference with a licensed provider for all active clients at least every 6 months.

Transitions clients out of medical case management into other appropriate programs as needed.

Completes change of status after meeting goals (stable) and transfers case into supportive case management or closes as appropriate.

If medically indicated, refer to DRS services; maintain client on caseload until stable.

Refer clients to educational support groups when appropriate.

Adheres to social services documentation policies and maintains documentation in the electronic medical record and other databases.

Ensures all transportation, vouchers, and related expenses are documented with client signature where applicable.

Tracks client/service trends and reports in monthly supervision.

Maintains knowledge of community and internal programs to support ongoing care.

Participates in mandatory trainings and meetings; aims to complete AFC Medical Case Management Certification within the first 6 months of employment and maintain certification.

Provides excellent customer service and maintains open communication with supervisors.

Performs other duties as assigned.

MINIMUM QUALIFICATIONS

Associate’s degree in related human service, social science, or health education with two years of experience working with LGBTQIA and/or HIV/AIDS clients with multiple needs, or a Bachelor’s degree with relevant experience.

Proficiency using Microsoft Office including Access and similar database programs.

Must obtain and maintain AFC Medical Case Management Certification within first 6 months of employment.

Bilingual in Spanish is required (Spanish bilingual testing will be administered; 80% minimum score).

PREFERRED QUALIFICATIONS

Three years of experience working with HIV/AIDS clients with multiple needs.

Licensed LSW or LCSW.

Experience in a primary care medical setting, hospital, or outpatient setting, and/or with clients with multiple needs.

KNOWLEDGE, SKILLS, ABILITIES, AND OTHER CHARACTERISTICS

Knowledge of HIV/AIDS and related issues; commitment to confidentiality and ethics; culturally sensitive counseling.

Excellent verbal and written communication and interpersonal skills for diverse populations.

Strong customer service and empathy skills.

Ability to work autonomously and as part of a team; adhere to policies and standards.

Maintain professional demeanor and handle confidential information appropriately.
