Responsibilities
The Adult Rehabilitative Mental Health Practitioner is responsible for home and community-based rehabilitative mental health services to individuals diagnosed with a mental illness and co-occurring disorders. Instruct, assist, and support recipients in areas such as interpersonal communication skills, community resource utilization, crisis assistance, relapse prevention, health care directives, budgeting and shopping, healthy lifestyle skills, cooking and nutrition, transportation, medication education and monitoring, symptom management, household management, employment-related skills, and transition to community living services. Ensure immediate and appropriate interventions are provided in response to changes in mental status, symptoms or behavior that may put the recipient at risk; updates to the Individual Treatment Plan as needed. Establish therapeutic relationships with assigned recipients and meet with each client as planned; maintain professional boundaries. Demonstrate cultural awareness and respect for differences in interactions with recipients and colleagues. Drive to client homes or community locations to provide services; occasionally transport recipients per supervisor direction. Complete initial and ongoing training requirements defined for the position to meet required standards. Prepare DLA-2 assessments, MHIS reporting, and develop individual treatment plans; complete annual crisis plans and revise documents as required. Develop and maintain recipient files per Minnesota statute requirements; document services accurately and within deadlines. Participate in discharge planning for recipients transitioning from Intensive Residential Treatment Services or inpatient settings when Transition Community Living (TCL) is authorized. Coordinate care and participate in collaborative treatment planning meetings, within CMMHC programs or with partner entities as applicable under the Certified Community Behavioral Health Clinic (CCBHC) framework. Participate actively in staff meetings and supervision. Requirements
Has at least 4,000 hours of supervised experience in delivering services to adults or children with mental illness, substance use disorder, or emotional disturbance; or Traumatic brain injury or developmental disabilities, plus training on mental illness, recovery, de-escalation techniques, co-occurring disorders, and psychotropic medications and side effects. Alternatively, may be qualified as a mental health practitioner through relevant coursework (30 semester hours or 45 quarter hours in behavioral sciences or related fields) with at least 2,000 hours of experience serving individuals with mental illness or substance use disorders, plus required additional training. Holds a master’s degree or other graduate degree in behavioral sciences or related fields. Holds a bachelor’s degree in behavioral sciences or related fields with completing a practicum or internship involving direct interaction with adults or children served, focused on behavioral sciences or related fields. Fluency in a non-English language spoken by at least 50 percent of clients, with completion of applicable training before providing direct services. Clear DHS Background Study and Valid Driver’s License with an acceptable driving record. What Will Set You Apart
