Join a growing healthcare team delivering in-home hospice visits across Will, Kane, Kendall, and DuPage counties. Contract PRN role starting at 10-14 hours weekly with growth to full-time potential. You'll manage your own schedule, chart remotely, and focus on direct patient care-not bureaucracy.

Most visits are local and planned around your own schedule-typically 3-5 patients per day, depending on need and travel time.

Your assessments directly determine patients' continued access to hospice benefits and comfort-focused care. What You'll Do

Perform face-to-face hospice recertifications in homes and facilities Evaluate and document hospice eligibility with IDG collaboration Prescribe and manage medications to control symptoms Consult with nurses and clinical staff for coordinated care Chart remotely using our EMR for accurate, timely documentation Work closely with the Director of Clinical Services on in-services or training needs Must-Haves Active Nurse Practitioner license in Illinois Active DEA registration Minimum 2 years' clinical experience (hospice, palliative, or related) Valid driver's license and insured vehicle Eligible to work in the U.S. Perks & Pay Pay: $60-$65/hour (contract, PRN) Flexible scheduling Mileage reimbursement Supportive clinical leadership Growth to full-time potential Schedule & Setup 10-14 hours weekly to start Flexible scheduling to meet certification timelines Biweekly IDG meeting (~2 hrs) Travel within Will, Kane, Kendall, DuPage counties Cloud-based EMR for remote charting
