PrecisionCare Responsibilities: ReviewsMD's referral and persons receiving services medical record prior to determining speech/language therapy services. Refers to specialty clinics as needed (ACC, dysphagia) Evaluate and assess the persons receiving services' needs and level of functioning in the areas of: speech, language, cognitive & social aspects of communication, voice, augmentative and/or alternative communication (ACC), and eating /management of diet (dysphagia) Devise treatment plans based on the assessment; administer treatment involving the application of communication and/or feeding aid modalities Write specific, objective, and measurable documentation which is outcome oriented and details the persons receiving services' needs, potential to benefit from treatment and progress achieved to date Communicate effectively with discharge planning from rehabilitative programs Perform all other duties as assigned. Qualifications: Graduate of an accredited Speech-Language Pathology school Master's Degree NYS Language Pathologist (SLP) license ASHA Certificate of Clinical Competence Solid understanding the principles of modalities employed in SLP Knowledge in the use of Augmentative Communication devices, Therapeutic Feeding techniques, and Speech & Language Intervention strategies Computer savvy Good written-verbal communication skills ***Must be able to travel as assigned to other Healthcare Centers, Article 16, day programs, or residential sites, if warranted.***
