About Upward Bound House UBH ends intergenerational family homelessness through crisis housing, permanent housing, and prevention services. For over 30 years, we’ve led with compassion, equity, and collaboration in a fast-paced, mission-driven, and growth-minded environment.

Role Summary The Case Manager, Thriving Families supports families with minor children as they transition into and stabilize in permanent housing. You’ll deliver trauma-informed case management, coordinate services, and partner with landlords to expand housing options and resolve issues that arise.

What You’ll Do

Complete comprehensive assessments and create Family Service Plans.

Provide ongoing case management: referrals, resource navigation, counseling, and wellness checks.

Provide field-based services, including travel to multiple service locations.

Complete required monthly home visits to support case management.

Lead the housing process end-to-end (unit search, landlord outreach/negotiation, applications, leasing).

Maintain accurate, timely documentation in HMIS and internal systems.

Conduct unit habitability/safety checks.

Mediate and resolve housing issues (late rent, habitability, neighbor concerns) with participants and landlords; collaborate with the housing stabilization team.

Build partnerships with owners/property managers and educate on rental assistance programs.

Coordinate with community partners to support long-term housing stability.

Participate in staff/case reviews, trainings, and community meetings.

Other duties as assigned.

What You Bring

Bachelor’s degree and 2+ years in human services (or equivalent relevant experience).

Experience working with families experiencing homelessness; strong knowledge of fair housing laws.

Trauma-informed, culturally responsive practice; clear professional boundaries.

Excellent communication (written/verbal), problem-solving, and conflict resolution skills.

Comfort working both independently and on teams in dynamic, fast-changing settings.

Proficiency with Microsoft Office and data systems (HMIS preferred).

>Valid CA’s license, reliable transportation, and ability to work occasional evenings/weekends.

Requirements

Employment eligibility verification and background check.

Current TB test.

Proof of auto insurance.
