Job Title: Family Resource Advocate (Resource Specialist)

Division/Program: Adult Full Service Partnership

Starting Compensation: 22.00 - 28.00 USD Per Hour

Working Location: Community-based in Fresno County

Working Hours/Shift: Monday–Friday, flexible hours based on client availability; typically within regular business hours (8:00 AM–5:00 PM)

What you bring to SBHG:

Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience)

OR

Bachelor’s degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience)

Valid California Driver’s License required.

How you will make a difference:

Conduct a housing screening on all participants involved in the FSP Program.

Develop an intervention strategy based on the needs of the client.

Incorporates the Transition to Independence Process (TIP) into services.

Advises Peer Specialist/Family Advocate, Mental Health Specialists, and Personal Service Coordinators on housing and resource options for families and participants.

Collaborates with program staff to ensure housing, education/employment, and resource needs are met.

Attends Team meetings, as needed.

Documents all services provided clearly and concisely within the Electronic Health Record within the timelines and framework established by Central Star.

Coordinates community contacts for events and services to be brought into the program. Develops and maintains a list of available resources accessible to all team members and families.

Participates in all outreach functions, community meetings, and family meetings as assigned.

Intervenes with the client in an emergency crisis to provide support and assistance in problem resolution.
