The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. The Investigator IV, Lead Investigator, performs highly complex (senior-level) investigative work at the State Supported Living Center (SSLC). Works under the supervision of the Incident Management Coordinator (IMC) or the Director of Incident and Risk Management (DIRM). Serves as the SSLC\'s Lead Investigator to ensure reports of unusual incidents including serious injuries and allegations of resident abuse, neglect or exploitation (ANE) are reported to Regulatory, HHSC State Office, and other agencies within required timeframes and investigated in accordance with agency policy and protocol. Upon notification of a qualifying incident, follows incident management protocols, coordinates immediate actions, conducts investigations, evaluates and analyzes documents and evidence, prepares and reviews reports, and communicates findings to supervisory staff. Enters, maintains, and tracks data; makes and follows up on recommendations. Works with internal and external entities to ensure qualifying cases are referred for investigation by Regulatory, law enforcement and/or the Office of Inspector General (OIG). Provides oversight, coaching, on-the-job training and mentoring of entry and journey level investigative staff. Serves as back-up IMC/DIRM as needed for coverage and business continuity. Attends meetings, participates in audits and inspections, and performs other duties as assigned. Works under limited supervision with considerable latitude for initiative and judgment. Serves in a on-call capacity and/or rotating schedule to assist with emergency investigation work, as needed. Essential Job Functions (EJFs) Attends work on a regular basis and may be required to work a specific shift schedule or rotating schedule, extended shift and/or overtime in accordance with agency leave policy; performs other duties as assigned. Serves as the SSLC\'s Lead Investigator to ensure reports of unusual incidents including serious injuries and ANE are reported to Regulatory, HHSC State Office, and other agencies within required timeframes; conducts investigations following agency policy and protocol. Collects and reviews evidence and data through field investigations and data collection, including direct observations and review of video surveillance; interview witnesses and complainants; collaborate with nursing, behavioral health, habilitation therapies to gather information; evaluate and summarize findings; prepare reports and present findings to supervisory staff. Prepares cases for presentation and provides testimony at administrative hearings or court proceedings if called. Assists the IMC as needed to ensure case assignments and coverage; serves as back-up IMC/DIRM for continuity. Coordinates incident actions, notifies required parties, and ensures necessary documentation and communications are completed; participates in the critical incident team (CIT) when needed. Provides training and mentoring of investigative staff; trains new investigators and develops investigative techniques in line with subject matter experts. Collaborates with external entities (Regulatory, local law enforcement, OIG) and ensures investigations progress without undue interference with concurrent internal investigations. Reviews and coaches others on report writing; tracks actions to completion; maintains data for management reports and regulatory submissions. Participates in audits and inspections; reviews policies and regulations for improvements; coordinates with SSLC State Office Incident Management. Acts as a member of the Incident Management Team, conducts Root Cause Analysis meetings, and participates in committees; contributes to prevention strategies and policy updates. Supports disaster response, continuity of operations (COOP) activities, and may require alternate shift patterns. Qualifications

Knowledge, Skills and Abilities (KSAs): Knowledge of state and federal laws and ICF-IID rules related to State Supported Living Center, or ability to gain that knowledge. Knowledge of laws and actions related to protection from harm and residential rights, or ability to gain that knowledge. Knowledge of investigative principles, techniques and procedures. Skill in the use of computer software including Word, Excel, Outlook and incident management systems. Skill in interviewing, gathering facts, and searching records to retrieve information. Skill in forming comprehensive investigative reports and in critical thinking, problem solving, research, and evaluating findings. Ability to convey thoughts clearly and to interview witnesses with varying emotional, educational, cultural, and social backgrounds. Ability to testify in hearings and court proceedings; ability to train, coach, mentor, and supervise others. Ability to plan, organize, and conduct investigations including death investigations; interpret and apply laws and regulations; work collaboratively with other investigators. Registrations, Licensure Requirements or Certifications:

None Education and Experience

Initial Screening Criteria: Graduation from an accredited high school or equivalent required; graduation from an accredited four-year college or university preferred. Experience in investigative work (planning, coordinating, leading or conducting investigations) or in quality assurance reviews. Experience in writing reports. Additional Information: Applicants must pass fingerprint background check, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide and LEIE). Selective Service registration for males ages 18-25 where applicable. All State Supported Living Center Employees are subject to Random Drug Testing.
