This is a full-time, day-shift role with potential on-call responsibilities and occasional overtime or weekend work. The position supports investigations of unusual incidents, serious injuries, and allegations of resident abuse, neglect, or exploitation (ANE) in accordance with agency policy and regulatory requirements.

Responsibilities
Serve as the SSLC Lead Investigator, ensuring reports of unusual incidents and ANE are reported to Regulatory, HHSC State Office, and other agencies within required timeframes and investigated per policy and protocol.
Conduct investigations, collect and analyze evidence, prepare investigative findings, and communicate results to supervisory staff.
Coordinate with internal and external entities (nursing, behavioral health, habilitation therapies, Regulatory, law enforcement, OIG) to gather information and ensure proper case handling.
Provide oversight, coaching, and on-the-job training for entry and journey-level investigators; assist the IMC/DIRM with case assignment and back up coverage as needed.
Attend meetings, participate in audits/inspections, and perform other assigned duties; operate under limited supervision with discretion and independent judgment.
Participate in on-call rotations and critical incident team activities to support emergency investigations.
Lead root cause analyses and facilitate corrective actions to prevent recurrence; review policies and regulations for continuous improvement.
Maintain primary contact with external entities for investigations and coordinate with Regulatory and OIG to ensure compliant, timely investigations.
Prepare testimony and evidence for administrative hearings or court proceedings when necessary.
Maintain data quality for management reports, including UIRs and incident tracking, and ensure timely documentation and follow-up actions.
Knowledge, Skills and Abilities (KSAs)
Knowledge of state and federal laws and ICF-IID rules, and related policies and procedures.
Knowledge of investigative principles, techniques, and procedures.
Proficiency with computers and software (Word, Excel, MS Outlook) and incident management systems.
Strong interviewing, data collection, and report-writing skills; ability to convey findings clearly in writing and verbally.
Ability to interpret and apply laws/regulations; collaborate with investigators and external agencies; testify in hearings or court proceedings if needed.
Ability to train, coach, mentor, and oversee others; plan, organize, and conduct investigations including death investigations.
Registrations, Licensure Requirements or Certifications
None
Initial Screening Criteria
Graduation from an accredited high school or equivalent required; graduation from an accredited four-year college or university preferred.
Experience in investigative work (planning, coordinating, leading, or conducting investigations) or experience in quality assurance reviews.
Experience in writing reports.
Additional Information
Applicants must pass fingerprint background checks, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, LEIE).
Selective Service registration may apply for males aged 18–25.
