Medical Social Worker - Integrated Care - Weekend Only at Dayton Children's Hospital Responsibilities
The Medical Social Worker is responsible for working in a team structure with the RN Case Manager and other Integrated Care department staff to coordinate the transition of care and complex discharge planning. Through psychosocial assessment and intervention strategies, assist patients and their families in understanding and coping with illness and disability. Help families resolve personal and environmental difficulties that interfere with social functioning and maximization of benefit from medical care. Participate as a professional team member in patient care planning with hospital and community agency personnel. Provide education and consultation to health care professionals and the community regarding psychosocial factors. Deliver social work services to the patient, the patient’s family, and others significant to the patient in a well-organized, integrated manner with hospital units and community partners. Collaborate interdepartmentally across inpatient, ambulatory, and Emergency divisions. Social Workers may also support onboarding, training, and mentoring for newer staff and students, and participate in the department’s peer mentoring program and special projects as requested. Qualifications
Experience:
1-2 years working within Healthcare or Hospital preferred Skills:
Professional skills in operating technology and providing customer service. Education:
Masters: Social Work (Required) Certification/License:

Job Details:
Full time; 34 hours per week; Weekend only Typical Schedule:
Friday 2:00 pm – 12:30 am, Saturday and Sunday 11:00 am – 11:30 pm. Eligible for premium weekend pay and shift differential. Facility and Department
