Medical Social Worker – Integrated Care
Dayton Children’s Hospital – Main Campus
Department: Social Work
Schedule: Full time, 40 hours per week (Monday – Friday, 8:00 AM – 4:30 PM)
Location: Pediatric inpatient floors – inpatient specialty pediatric care
Responsibilities
Collaborate with the RN Case Manager and other Integrated Care department staff to coordinate overall care for transition of care and complex discharge planning.
Conduct psychosocial assessments and develop intervention strategies to assist patients and families in understanding and coping with illness and disability.
Assist families in resolving complex personal and environmental difficulties that interfere with effective social functioning and maximize benefits from medical care.
Participate as a professional team member in patient care planning with hospital and community agency personnel.
Provide education and consultation to health care professionals and the community regarding the impact of psychosocial factors.
Deliver social work services that are organized, directed, and integrated with other units and departments of the hospital and community.
Collaborate interdepartmentally with all divisions of the hospital, including inpatient, ambulatory, and emergency services.
Provide additional support with onboarding, training, and job shadowing for newer staff and students.
Participate in the department’s peer mentoring program, develop social work skills, and complete additional special projects as requested.
Qualifications
1–2 years of experience working within healthcare or hospitals preferred.
Professional skills in operating technology and providing customer service.
Master’s degree in Social Work (Required).
Ohio Licensed Social Worker (LSW) – Counselor, Social Worker or Marriage & Family Therapist Board.
