Position Type:

Full Time.

Salary:

$22.00 - $24.00 per hour.

ISP Coordination and Development

Lead the development and maintenance of each individual’s ISP in collaboration with the interdisciplinary team.

Ensure all ISP documentation accurately reflects the individual’s strengths, goals, preferences, and support strategies.

Schedule and facilitate annual and interim ISP meetings, ensuring timely completion and submission of all required documentation.

Update and monitor progress on ISP goals throughout the year, coordinating revisions as needed.

Ensure ISPs align with BHDDH, Medicaid, and agency requirements.

Residential Support and Oversight

Collaborate with Residential Managers and Direct Support Professionals (DSPs) to ensure daily supports align with ISP objectives.

Monitor implementation of support strategies within the home environment and provide follow-up coaching to staff when needed.

Ensure that data collection and progress notes accurately reflect services provided and progress toward goals.

Participate in quality assurance reviews and respond to corrective action items related to ISP implementation.

Communication and Collaboration

Serve as the primary contact for all ISP-related matters for assigned individuals.

Communicate regularly with families, guardians, and external case managers to ensure continuity of care and satisfaction with services.

Facilitate effective communication between residential, day, and clinical teams.

Participate in interdisciplinary meetings, trainings, and case reviews.

Documentation and Compliance

Maintain accurate and timely documentation in Therap and other systems as required.

Track ISP due dates, revisions, and action items to ensure compliance with all timelines.

Support audits and reviews conducted by BHDDH, funding sources, or agency leadership.

Ensure all consent forms, assessments, and required attachments are current and properly filed.

Qualifications

Associate’s degree in Human Services, Psychology, Social Work, or related field required.

Minimum of 2 years of experience working with adults with intellectual and developmental disabilities preferred.

Strong knowledge of ISP process, person-centered planning, and BHDDH regulations.

Excellent communication, organizational, and time-management skills.

Proficiency in Therap or other electronic health record systems preferred.

Ability to work collaboratively across departments and independently manage multiple priorities.
