AccessPoint RI . We are currently looking for a Licensed Practical Nurse to assist our personal choice and Residential Program. We are seeking an LPN who is available 2 or 3 days per week. Location: Cranston, RI 02920.

Essential Duties And Responsibilities
Provide competent professional medical services to individuals receiving day and residential services.
Deliver care to individuals using the nursing process of assessment, planning, intervention, implementation, and evaluation.
Interact with individuals and their families, clinicians, and direct support staff effectively and professionally.
Answer phone calls and document information as necessary.
Contact providers for questions, updates, or concerns as needed.
Notify clients’ families of incidents.
Visit clients in group homes as needed.
Attend staff meetings as needed.
Coordinate services with the Physical Therapist, Occupational Therapist, and Speech Therapist as needed.
Ensure bills are paid accurately and on time.
Annually evaluate individuals’ medical conditions and their impact on daily functioning.
Assist individuals in identifying and accessing available community resources related to wellness and health promotion and/or maintenance.
Report health and safety concerns to appropriate state agencies if exploitation, abuse, or neglect criteria are evident.
Serve as a liaison between AccessPointRI and state or other agencies providing support for individuals, in conjunction with the Residential Director.
Perform all duties as needed.
Qualifications
Current unrestricted Rhode Island LPN license.
Minimum 2 years of nursing experience; experience in community health, home care, or long-term care preferred.
Strong assessment and documentation skills.
Ability to manage a large caseload with efficiency and attention to detail.
Knowledge of Medicaid/Medicare and self-directed service programs preferred.
Excellent communication and interpersonal skills with individuals, families, and interdisciplinary teams.
Valid driver’s license and reliable transportation for statewide travel.
