Social Worker - Medical/Dialysis (MSW) role at Inside Higher Ed. This position is located at The Ohio State University (Rhodes Hall). Responsibilities
Provide psychosocial assessments, mental health counseling, crisis intervention, discharge planning, health education, and linkage to community resources. Address medically related social and emotional needs of patients and families to support treatment, recovery, and safe transitions between care environments. Confer with renal patients, the patient’s physician, discharge planners, social workers, and hospital staff regarding post-discharge plans, including start of outpatient dialysis, transition to outpatient dialysis, referrals to nephrologists, and vascular access information. Minimum Qualifications
Master of Social Work from an accredited Graduate School of Social Work. State of Ohio Social Work licensure (LSW). Additional Information
