Overview

As the Crisis Services Intake Discharge Coordinator you’ll be a valued member of a creative and compassionate clinical team with a track record of delivering outstanding results. In this role, you will provide screening, assessment, and referrals to the appropriate Crisis Services program in order to decrease unnecessary emergency department visits and hospitalizations for youth in Dutchess County. You will work closely with the Home Based Crisis Intervention and Partial Hospitalization teams to screen referrals and conduct intake assessments. Additionally, this position provides an integral role in discharge planning and ensuring continuity of care by linking youth and families to appropriate community resources. The Intake Discharge Coordinator is part of a team that ensures 24-hour continuity of clinical coverage to youth and families enrolled crisis services. Astor Crisis Services provides access to high-quality clinical supervision, being a part of supportive team and opportunities for training in evidence-based practices, including Dialectical Behavior Therapy (DBT).

Responsibilities

Provide screening, assessment, and referrals to the appropriate Crisis Services program to decrease unnecessary emergency department visits and hospitalizations for youth in Dutchess County.

Work closely with the Home Based Crisis Intervention and Partial Hospitalization teams to screen referrals and conduct intake assessments.

Play an integral role in discharge planning and ensure continuity of care by linking youth and families to appropriate community resources.

Be part of a team that ensures 24-hour continuity of clinical coverage to youth and families enrolled in crisis services.

Qualifications

LCSW, LMSW and LMHC will be considered based on experience and diagnostic privilege.

Valid Driver’s license that meets agency standards.
