Assistant Program Manager (DDS/Millennium House) at The Salvation Army USA Eastern Territory. The agency pursues a fully integrated system of care, requiring holistic assessment of needs and connection to internal or external services as needed. Responsibilities

Organize and maintain up-to-date records. Participate in Interdisciplinary Team meetings and staff meetings. With the Program Manager, create meeting agendas and plan standard items (reports, announcements, staff concerns). Collaborate with consultants, especially Psychologists, on programming. Follow through on recommendations for U.R. committee; attend U.R. meetings when delegated. Complete and maintain incident investigations related to the DDS program; ensure life-safety safeguards are followed. Maintain timely and accurate records and reports on health, safety, and program quality; submit and close incidents per policy; comply with OPWDD and Justice Center guidelines. Make records available for review for IPR & UR; respond to referrals and follow up as needed. Assess the needs of individuals and their families, including basic needs (food, clothing, shelter); coordinate referrals to internal/external services. Prepare staff schedules for Program Specialists; manage vacation, personal, sick leaves and coverage during shortages; ensure appropriate staffing patterns. Conduct supervisor meetings to discuss programming and staffing; evaluate staff performance and provide counseling. Support skill development in activities of daily living, domestic living, recreation, and socialization. Assist Program Manager in developing and implementing agency/program policies per OPWDD or funding guidelines. Conduct rounds across shifts to ensure safety, health, environment quality, and program delivery; communicate findings to Program Manager. Perform medical assistant duties: track appointments, coordinate with CMS, nurses and doctors; manage medication inventory and referrals; maintain medical records. Coordinate transportation to maximize program participation and accompany individuals to programs, appointments, and activities as needed. Ensure completion of Staff Information Entry forms when escorting individuals to medical appointments; ensure availability of essential items (toiletries, clothing, personal effects). Prepare Day Program progress reports for ITT reviews as assigned; manage petty cash and reimbursements. Conduct monthly fire evacuation drills with all staff on all shifts. Participate in admission and discharge reviews and provide recommendations; develop monthly recreation schedules in consultation with Program Manager. Implement and monitor in-house and community recreational activities with hands-on support as needed. Other duties as assigned; direct care as necessary. Qualifications

High school diploma with one year of experience in Human Services or related field required; Associate’s Degree preferred. 1 year administrative experience. 2 years minimum experience working with individuals diagnosed with intellectual disability (ID). Respectful and considerate toward residents; AMAP, SCIP-R and First Aid/CPR certification. Valid driver’s license; ability to drive large vehicles (wheelchair or 15-seater van). Additional details
