Requirements to Apply:
Bachelor’s degree in Physical Therapy from an accredited program
Current California Physical Therapist license
Current CPR certification
At least 1 year of experience working with a frail or elderly population (clinical placement may substitute)
Valid California driver’s license with good driving record; ability to travel for home visits

Job Overview
We’re seeking a dedicated Physical Therapist to join our interdisciplinary team (IDT) and provide high-quality therapy services to seniors in our PACE program. The ideal candidate is compassionate, collaborative, adaptable, and skilled in both clinical practice and patient-centered care. Key Responsibilities
Complete initial, semi-annual, annual, and unscheduled participant assessments; report changes to IDT promptly Develop, implement, and monitor therapy care plans, including restorative programs Provide treatment sessions, evaluate progress, and adjust plans as needed Recommend, order, and support use of adaptive and durable medical equipment (DME) Train and supervise restorative aides, PT assistants, and family/caregivers on therapy programs Educate staff and participants on body mechanics, equipment, and therapy-related practices Perform home visits as required, ensuring participant safety and independence Maintain accurate documentation and participate in QAPI initiatives and team meetings Support program development and communicate staffing or service needs to leadership Benefits
