Job Summary and Responsibilities The Rehabilitation Admissions Coordinator performs activities related to pre‑admission evaluation and admission of patients to the inpatient rehabilitation unit. Their goal is to identify patients who meet program criteria and will benefit from treatment in an acute rehabilitation program.

Key responsibilities include:

Reviewing medical records to complete the pre‑admission screening tool and determine the appropriateness of the patient for admission.

Collaborating with medical directors and case management to facilitate timely transfers.

Ensuring benefit authorization is completed as required.

Coordinating bed and team assignments in partnership with nursing and other staff.

Building relationships with external referral sources.

Gathering and tracking data for planning and ensuring compliance with regulatory/accreditation requirements.

Minimum Requirements

Two years experience as an RN.

Associate degree in Nursing.

RN license in AZ or COMPACT state.

Preferred Qualifications

Previous experience as an Admissions Coordinator or Rehab Coordinator.

Bachelor's Degree in Nursing.

Pay Range $39.18 – $58.28 /hour
