ACT Case Manager (Full Time) Location: Woonsocket, RI

Our Assertive Community Treatment Team is looking for a Case Manager who has experience working in the substance use and mental health field. You will be an integral part of our multidisciplinary team to help people avoid crisis situations and keep crises from turning into unnecessary hospitalizations. ACT is designed to help those individuals who are struggling with serious and persistent mental illness and co-occurring substance use disorders that may need more than traditional outpatient mental health care and psychiatric rehabilitation. In this role you will provide comprehensive community support to individuals living in their homes, stabilize clients through ongoing assessments, offer support services and crisis interventions, develop and implement treatment plans with clients to promote recovery, and collaborate with community resources to enhance mental health and rehabilitation. This is a full‑time position with benefits.

Education / Licensing Requirements

BA/BSW in a human services related field required.

Knowledge of substance abuse and mental health is a plus.

Experience with Electronic Health Record systems is preferred.

Approved driver’s record and reliable transportation required.
