We are seeking a

Social Worker (LCSW)

to join the Palliative Care team. The role of the PACT Social Worker is to support PACT inpatients and outpatients in partnership with the interdisciplinary palliative care team. The PACT social worker partners with the primary disease / unit based social worker and psychosocial providers to facilitate family communication with the medical team. The PACT social worker joins the Palliative Care IDT to engage in family decision-making and goals of care discussions, assist with developing an end of life care plan as appropriate, initiate / support hospice referrals and discharge coordination, and support transition to bereavement when indicated. The PACT social worker is responsible for maintaining EMR documentation, participates in division quality improvement activities, is able to participate in research activities if interested, and other duties as assigned. The Nemours Palliative Care Team (PACT) seeks a social worker to support Palliative Care inpatients and outpatients in partnership with the interdisciplinary palliative care team. The PACT social worker partners with the primary disease / unit based social worker and psychosocial providers to facilitate family communication with the medical team. Responsibilities: Partnership with PACT psychosocial partners: bereavement coordinator, chaplain, child life specialist Partner with primary disease / unit based social worker and psychosocial providers Facilitate family communication with the medical team Conduct bio-psychosocial-cultural-spiritual assessments for families Engage in decision-making and goals of care discussions Follow patients in inpatient and outpatient settings Documentation in EMR Develop end of life care plans as appropriate Initiate/support hospice referrals and discharge coordination Maintain communication with hospice social workers if applicable Coordinate postmortem family conferences Serve as Gift of Life Partner for PACT Support clinical staff in psychosocial aspects of care Perform additional duties as assigned by supervisor Qualifications: Master's degree required LCSW with Delaware licensure required APHSW-C (Advanced Palliative Hospice Social Worker – Certified) preferred Minimum of three years' experience in multidisciplinary teams preferred #LI-AE1
