Pay Range This role offers a base hourly pay range of $30.00/hr - $45.82/hr. Your actual pay will be based on your skills, experience and qualifications.

Responsibilities

Develop, plan, implement and evaluate psycho/social interventions for patients in Home Health, Hospice IP/OP, and Palliative programs.

Identify patients with complex psycho/social needs, assess eligibility for social programs and community assistance.

Provide crisis intervention, psychoeducation, supportive counseling and bereavement support.

Coordinate with interdisciplinary team members to develop patient plans of care.

Facilitate discharge planning, family meetings and post-acute linkage.

Supervise and mentor social workers in practicum settings.

Serve as the hospice volunteer coordinator, manage volunteer training, background checks and community outreach.

Assist the hospice chaplain and develop pain and symptom management plans.

All other duties as assigned by supervisor.

Qualifications

Masters degree in Social Work required.

Current license as Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Master Social Worker (LMSW) in New Mexico.

10 years hospital social work or discharge planning experience for Social Worker III; 5 years for Social Worker II; 1 year for Social Worker I in relevant settings.

National Case Management certification or Palliative Care certification preferred (ACHP‑SW, APHSW‑C).

Experience in case management and palliative care settings.
