We are seeking an experienced SNF Staff RN who is responsible and accountable for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at Seton Medical Center and applicable federal and state regulations. Responsibilities
Assess, collect and document health data focusing on the physical, emotional, spiritual, psychosocial and educational needs of the patient. Interview the patient/family/significant other to complete admission information; identify actual and potential needs and determine priorities; identify barriers to learning. Complete and document assessments, plan nursing care based on priority of physical and psychosocial-emotional needs, and perform ongoing assessments as indicated. Develop realistic short-term goals for each problem, involving the patient, family or significant others; plan demonstrating understanding of age-related needs; consult with patient, family, significant others and health care team members for education and discharge planning. Implements the plan of care including prescribed procedures and treatments; organize and prioritize care; assess changes in patient condition and appropriateness of the plan; initiate interventions using clinical expertise; coordinate activities of health care team members. Evaluate the effectiveness of nursing care and document patient status and response to care. Provide health education and teaching to patients, family members and/or significant others that is age-specific and addresses barriers to learning; clearly communicate discharge instructions and follow-up needs. Perform procedures within scope of practice and facility policies; recognize and respond quickly to emergency medical situations (ABCs, CPR, oxygen, etc.). Remain current on treatments, practices and procedures; integrate new knowledge from education and research into practice; demonstrate competence in age-related patient care and adherence to state and federal regulations. Direct, oversee and evaluate nursing personnel; guide staffing procedures; set objectives and long-term goals for the nursing department; organize admission, nursing and patient care processes; manage record-keeping; collaborate with other departments to streamline operations. Report to Director of Nursing; participate in decisions regarding nursing care and coordinate educational offerings for the nursing department. Participate in Medicare and Insurance assessments and complete required worksheets; ensure nursing department documentation aligns with facility protocol. Education
Graduation from an accredited nursing school preferred; Bachelor’s degree in Nursing preferred. Certifications/Licensure
Current California licensure as a Registered Nurse. Current Basic Life Support (BLS) training through the American Heart Association. Vaccination: Covid-19 vaccination recommended and up to date. About AHMC Seton Medical Center
