Overview
We are looking for an experienced SNF Staff RN, a professional caregiver responsible for assessing, diagnosing, planning, implementing, and evaluating patient care in compliance with standards at Seton Medical Center, federal, and state regulations. Responsibilities
Clinical Practice: Nursing Assesses, collects, and documents health data focusing on physical, emotional, spiritual, psychosocial, and educational needs. a) Conducts interviews with patients, families, or significant others to gather admission information. b) Identifies actual and potential needs and sets priorities. c) Recognizes barriers to learning. Completes ongoing assessments, focusing on abnormal systems and potential issues. Plans care based on priorities, involving patients and families in goal setting, considering age-related needs, and coordinating education and discharge planning. Implements care plans, prioritizing tasks, assessing patient responses, and coordinating team activities. Evaluates care effectiveness, adjusting plans as needed, and documents patient status and responses accurately. Clinical Practice: Progressive Care Unit
Utilizes professional knowledge to assess physical and psychosocial needs, barriers to learning, educational needs, discharge needs, age-related issues, and issues related to assault or abuse. Analyzes patient data to determine appropriate care and applies advanced skills for patient-centered interventions. Provides age-specific health education, ensuring understanding of treatment and discharge instructions. Performs procedures within scope of practice, recognizing emergencies, and initiating appropriate responses like CPR and oxygen therapy. Stays current with best practices through ongoing education, integrating new knowledge into clinical practice, and demonstrating competence in age-related patient care and regulatory compliance. Manages nursing personnel, guides staffing, sets departmental goals, organizes care procedures, manages documentation, collaborates with other departments, reports to the Director of Nursing, and oversees nursing standards and education programs. Qualifications
EDUCATION: Graduation from an accredited nursing school preferred; Baccalaureate degree preferred. Complete Plan of Correction knowledge. CERTIFICATIONS/LICENSURE: Current California RN license, Basic Life Support training through AHA, recommended COVID-19 vaccination. Hospital Description
