Position Information

Facility: (Center/Building Name)
Practice Setting: (Short Term Rehab, Assisted Living, LTACH, Long Term Care)
Essential Duties and Responsibilities
Provide and direct resident rehabilitation care using general accepted practice methods and meeting all center, state, federal, and accrediting standards.
Evaluate residents’ needs and establish effective treatment plans for assigned residents.
Manage and coordinate the supervision of treatment plans in collaboration with all staff and other disciplines.
Participate in resident screening per directed schedules and practices.
Provide feedback to the designated supervisor on all clinical and rehabilitation issues.
Participate in all center‑directed processes and services.
Perform necessary clinical documentation to meet all regulatory requirements.
Maintain complete, accurate, and timely billing records in accordance with center, state, federal, and accrediting agency requirements.
Participate in in‑service training programs as requested.
May travel to other facilities to provide services on a limited basis.
Position Requirements
Master’s or Doctorate Degree in Physical Therapy.
Current and active Physical Therapist license in the state of operations.
Preferably experience or knowledge in older adult rehabilitation.
Preferably knowledge of governmental and regulatory guidelines in designated healthcare settings.
