Case Manager Location: 337 E Virginia Ave, Phoenix AZ 85004 Hours: Mon-Fri 8am - 5pm Position Summary

Under the direct supervision of the Clinical Supervisor, the Case Manager ensures coordination and continuity of care. Provides skill training and development, intake assessment, facilitates groups, connects clients to community resources, and provides coordination of care. Responsibilities

Completes initial intake/assessments for new clients and completes all related paperwork in a timely manner. Assists with or facilitates health promotion, psychoeducational and skills training groups; provides individualized case management to Outpatient (Integrated Health) clients. Updates treatment plans for clients, as needed. Completes clinical documentation on a daily basis including progress notes, intake and discharge documentation. Assists with development of individual and client cultural competencies. Addresses Social Determinants of Health needs with documentation (e.g., workforce development, resumes, interviews as indicated by client need). Referrals for basic needs and supplies (medical appointments, eyeglasses, etc.). Referrals for housing, employment, job training, etc. Assists with transportation resources as needed. Assists in obtaining benefits such as applications for food stamps, GA, AHCCCS. Assists in making community referrals and support. Assists clients with housing interest forms and VI-SPDAT. Coordinates care among treatment team, referral sources, and multidisciplinary teams, including Adult Recovery Team meetings. Provides coverage for Front Desk as needed. Supports and actively engages in inclusive practices through Cultural and Linguistically Appropriate Services (CLAS) standards. Completes duties using trauma-informed care principles. Bachelor’s degree in behavioral health field preferred; or Associates of Arts Degree in behavioral health field and 2 years’ experience in licensed facility; or Associates of Arts Degree with LSAT AzBBHE license; or High school diploma or GED and 4 years’ experience in a licensed facility; or High school diploma or GED and 18 hours credit in related field and 2 years’ experience in a licensed facility; and Possess and maintain a valid Arizona driver’s license Fingerprint Clearance Card Skills

Excellent customer service skills. Critical thinking, problem-solving, organizational and time management skills. Proficiency in electronic health record systems Team-oriented collaboration Understanding of chemical dependency recovery processes Experience working with Native American populations (preferred) Knowledge of Native American economic, educational, and social issues and available referrals (preferred) MS Office proficiency Bilingual - Spanish desired Physical Demands
