GENERAL DESCRIPTION Performs journey-level to advanced and/or supervisory case management work to develop and maintain long-term contact with assigned youth, their families, and providers for medical, social, educational, and related service needs. Work includes case file reviews, case planning, family involvement and group service delivery, developing goals, and evaluating outcomes. Performs discharge and release functions to coordinate service delivery during the youth’s commitment in TJJD. May work in excess of 40 hours per week, with different shift patterns and on-call duties. Career Path Promotion Opportunity : This position is within the TJJD Case Manager career path and may promote up to Case Manager IV after meeting education, experience, and performance criteria per agency policy (PRS.15.07). Pre-Service Training Academy Requirement : Newly hired case managers or rehired case managers who separated more than 18 months must attend the pre-service training academy. Case managers returning within 18 months will be placed in full-time status on their date of employment. Levels of Work Performed by Career Path Level : Case Manager II:

Moderately complex journey-level work under general supervision with some initiative and judgment. Case Manager III:

Highly complex senior-level work under limited supervision with considerable initiative and judgment. Case Manager IV:

Advanced and/or supervisory senior-level work under minimal supervision with extensive initiative and judgment. ESSENTIAL DUTIES Conducts initial case assessment and file reviews, requests missing documentation, performs stage assessments, and conducts ongoing assessments for assigned caseload. Performs case planning, applies strategies to facilitate youth progress and referrals, monitors transition planning, conducts and facilitates required groups, and plans for leave to ensure group services. Participates in staff development and provides individual counseling. Documents services and maintains case files. Reads and applies youth records information related to health, safety, and facility security. Complies with policies and maintains records. Attends work on a regular schedule. Performs safety-related duties and assists with incident reporting and documentation. Minimum Qualifications Case Manager II – Pay Level 1 : Unrelated Bachelor’s degree; or High School diploma or equivalent plus 15 college hours and 1 year of relevant experience; or 2 years of related experience with less than 15 college hours. Other listed combinations of education/experience apply. Case Manager III – Pay Level 2 : Bachelor’s degree in a related field with no experience required, or related combinations of higher-level education and experience as detailed in the original description. Case Manager IV – Pay Level 4 : Master’s degree or Bachelor’s with related experience; or higher combinations as described. Note : Any degree, diploma, or hours to meet minimum qualifications must be from an accredited institution. Requirements for All Levels Must be at least 21 years of age. KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of child development, child care practices, case management delivery systems, and program policies. Skills in developing, implementing, and monitoring service plans; emergency responsiveness; working effectively with youth; record-keeping; maintaining order; prioritizing tasks; communicating effectively; guiding others; and building relationships.
