Overview Youth and Family Case Manager role at Bluebonnet Trails Community Services. Under the supervision of the Youth and Family Clinical Program Manager, the case manager provides recovery-oriented case management services, skill building, and medication education. Conducts psycho-social-emotional needs assessments, completes Person-Centered Recovery Plans, assists youth and families with accessing community resources, provides rehabilitative services, and initiates crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team.

Responsibilities

Provide recovery-oriented case management services, skill building, and medication education under supervision.

Conduct routine psycho-social-emotional needs assessments and complete Person-Centered Recovery Plans.

Assist youth and families with accessing community resources and providing rehabilitative services.

Initiate crisis intervention when needed and document all services in the electronic health record in collaboration with a multidisciplinary team.

Maintain high-quality documentation, adhere to company and state guidelines, and dedicate a minimum of 55% of hours to direct service.

Qualifications Minimum qualifications:

Bachelor\'s degree in Human Services or a minimum of 24 credits in a human services field if degree is not in Human Services; comprehensive knowledge of mental health diagnoses and treatment; ability to work independently and in a fast-paced, recovery-focused environment; valid Texas Driver\'s License.

If the degree is not in Human Services, a minimum of 24 credits in a human services field.

Possesses a valid Texas Driver\'s License and maintains approved status.

Preferred qualifications:

Master\'s degree in Human Services.

One year of experience in wellness and recovery support services in mental health, substance use, and/or crisis intervention and screening.

Pursuit of clinical licensure in counseling, behavior analysis, family therapy, social work, or a related field; advanced licensure; Licensure as a Licensed Chemical Dependency Counselor (LCDC).

Bilingual in ASL or Spanish; experience with concurrent documentation.

Location and Compensation

Location: Giddings, TX

Hourly salary range: $21.46 - $30.71

Differential details: Up to $4.00/hour for tenure with BTCS; $0.75/hour for bilingual; up to $2.00/hour for prior experience in a similar role with a Texas community center; $1.00/hour for a Master\'s Degree in Human Services; $1.00/hour for advanced licensure; $0.50/hour for LCDC licensure; eligibility for a $200/month Service Delivery Incentive Award.
