Facility Location : Shelby Health & Rehabilitation Center, 46100 Schoenherr Road, Shelby Township, MI 48315
Schedule : Full Time, Monday-Friday
General Summary In this role you will provide speech-language pathology services to patients with cognitive/communication or swallowing dysfunction, including assessment, treatment planning, and implementation. You will complete appropriate documentation and adhere to policies, procedures, licensure laws, and current standards of clinical practice.
Your Responsibilities
Conducts speech-language pathology screenings and evaluations to determine patient cognitive/communication and/or swallowing status using standardized tests and measures.
Develops and implements treatment plans to maximize independence in line with ASHA standards to improve and maintain function.
Assess effects of treatment and modify plans to achieve maximum benefit.
Supervises and reviews the work of CFY as assigned and in accordance with ASHA standards.
Collaborates with physicians and other practitioners to obtain patient information, revise treatment plans, and integrate speech therapy with overall health care.
Maintains documentation to ensure quality of the medical record, continuity of care, and reimbursement guidelines.
Participates in patient and family care conferences as requested.
Educates and instructs patients and families in treatment procedures and home exercise programs.
Participates in Quality Assurance activities and contributes to program development and agency meetings as directed.
Engages in continuing education and certifications to promote current clinical practice.
Required Qualifications
Master’s Degree in Speech-Language Pathology from an accredited program
ASHA Certificate of Clinical Competence
Licensure as required by the State of Michigan
