Summary: Under the supervision of Senior Clinical Dietitian Specialists and the Clinical Nutrition Director/Supervisor, provides nutritional care to hospital patients. Interviews patients/family, reviews electronic health records, assesses nutritional needs, and develops plans of care. Monitors and evaluates outcomes related to nutrition diagnoses, goals, preferences, and interventions to determine progress and whether interventions should be continued or revised. Documents patient progress and updates the nutrition plan in the electronic health record. Directs the activities of support personnel in assigned areas as needed. Responsibilities Use accurate data to identify nutrition-related problems. Assess current and past information related to personal, medical, family, and psychosocial history. Assess anthropometric indicators and compare to reference data to evaluate growth and patterns. Assess laboratory profiles, medical tests, and procedures related to nutrition and disease states. Obtain nutrition-focused physical examination findings and food/nutrition history, including medication and supplement use, beliefs, readiness to change, and factors affecting nutrition goals. Estimate nutrient needs and set target body weight/BMI and growth patterns as appropriate. Review data collected by dietetic staff and other practitioners to identify factors affecting nutrition status. Identify possible nutrition problems and revise diagnoses per standards of care. Base intervention/plan on evidence and guidelines; document plans in the Interdisciplinary Plan of Care and routine EHR documentation; monitor and update as needed. Participate in discharge planning from a nutritional perspective and recommend programs to optimize readiness for discharge. Provide individual and group nutrition education inpatient and outpatient; prepare patients and families for home care. Lead nutrition education seminars/workshops for physicians and other professionals; participate in community and interdepartmental activities. Represent Food & Nutrition Services at interdisciplinary meetings; participate in patient satisfaction initiatives and quality improvement programs. Support department financial and productivity measures as requested; may participate in community/media programs. Minimum Qualifications Bachelor’s Degree in Food and Nutrition or a related concentration from an ACEND-approved program. Completed ACEND-accredited supervised practice and passed the CDR national examination for Registered Dietitian Nutritionists (RDN), or provide documentation of scheduled registration exam date if eligible. Licensure as a Dietitian/Nutritionist in Rhode Island (must have within 3 months of hire). If unlicensed, notes cosigned by a licensed RD until license is obtained; proof of license application required on employment. Knowledge of nutrition principles as applied to adult and pediatric disease states; understanding of the nutrition care process and nutrition values of foods. Strong analytical, critical thinking, and communication skills; ability to develop nutritional plans and coordinate care delivery. Maintenance of CDR continuing education; RI LDN CE requirements; membership in professional groups preferred (e.g., AND MNPG/PNPG). Proficient computer skills (MS Word; MS PowerPoint and Excel preferred). Experience One to two years of experience as a Clinical Dietitian preferred. Work Environment and Physical Requirements Sits, stands, and walks intermittently; exposure to normal patient care environments. May lift up to 10 pounds; safety precautions followed. Travel may be required to off-site facilities.
